Once scheduled, if you would feel more comfortable doing your paperwork in the privacy of your home you may fill out our Intake Form before your appointment on your computer or by printing it out.

To complete it on your computer, click "enable editing" once the file opens, enter your information, save your edited version where you can find it, then email it to us through the "Reach Out" button below. Just click the paperclip symbol to add your intake form as an attachment.
